Memory Clinic
The Burlington Family Health Team (BFHT) is excited to launch its Memory Clinic using the MINT (Multispecialty INterprofessional Team) model to support people experiencing new or ongoing memory concerns. Our team of specially trained clinicians is made up of a Nurse Practitioner, Nurses, Occupational Therapists, a Pharmacist, a System Navigator,
Dr. Jocelyn Anderson, (specialized in care of the elderly), Dr. Junghyun Sarah Park, (consulting Geriatrician) and the Alzheimer’s Society.

Our interdisciplinary team will work closely with your family doctor to provide assessment, diagnosis, a treatment plan and support for patients with mild to moderate cognitive impairment and their support person. The Memory Clinic will ensure memory care is accessible, evidence-informed, comprehensive and focused on maintaining health and wellness of both patients and caregivers. The first clinic will be held on November 4-5, 2024.

To access the BFHT Memory Clinic a referral from your family doctor is required.

There is no cost to patients.

How do I get an appointment at the Burlington FHT Memory Clinic ?If you or a loved one are experiencing new or ongoing memory difficulties ask your family doctor or nurse practitioner working in Burlington for a referral to the Burlington Memory Clinic. All services are fully covered by your Ontario Health Insurance Plan (OHIP) and there is no additional cost to accessing this service. Burlington physicians and nurse practitioners can make an eReferral through OCEAN.

What happens after my first visit to the Burlington FHT Memory Clinic?For many memory conditions, early diagnosis and treatment can help maintain and support cognitive health and quality of care. Our goal is to help people living with memory difficulties maintain independent living and the best quality of life for as long as possible. After your first visit, the Memory Clinic team will continue to work closely with your family physician/nurse practitioner, community agencies and specialist physicians (if required), with the aim of providing coordinated, ongoing care for you and your family members.

What Is A MINT Memory Clinic ?A MINT (Multispecialty Interprofessional Team) Memory Clinic is a team of clinicians who are specifically trained to support individuals experiencing memory issues. The Burlington Memory Clinic team includes a physician, a geriatrician, Nurses, Occupational Therapists, Physiotherapists, Pharmacists, Dietitians and a System Navigator. The team has access to consulting specialists like a Geriatric Psychiatrist and Cognitive Neurologist and they work in partnership with community organizations like the Alzheimer Society Brant, Haldimand Norfolk, Hamilton Halton. The team provides assessment, early diagnosis, treatment and education in a primary care setting for people living with dementia and other memory disorders. What can I expect when I visit the Burlington FHT Memory Clinic?Once the Memory Clinic receives your physician’s referral, a clinic team member will contact you to arrange an appointment. You will also receive an information package with forms for you to complete and instructions on how to prepare for your appointment. Clinic appointments typically run between 2 to 4 hours. The Memory Clinic will provide you with lots of care and support information, so please bring along a family member or friend who knows you well and can stay for the duration of the appointment. This will help the team to provide a complete memory assessment. On the day of your appointment, you and your family members will meet with the team of skilled health care professionals who will complete a thorough memory assessment and provide recommendations on many aspects of memory function, including blood work and x-ray test that may be required, available treatments and driving safety. The information collected will provide you and your family doctor or nurse practitioner with a better understanding of your cognitive health and recommend steps you and your family can take to support you.
