Aging Well Clinic

The Burlington Family Health Team’s Aging Well Clinic is focused on prevention and self-management of health conditions to support older adults to age well in the community. Our team of Clinicians (a Registered Nurse, an Occupational Therapist, a Physiotherapist, and a Registered Dietitian) are trained to perform screening to identify risk factors associated with aging and well-being among older adults.

 

Our Aging Well Clinic is held at the following locations:

1. Every Friday: Burlington Family Health Team Guelph Line Location, 720 Guelph Line, Unit 204, Burlington

2. First Thursday of each month: YMCA Burlington, 500 Drury Lane, Burlington
